Medical errors can occur at any point in treatment, even in preventive care, and do not always result in patient injury or death. Dental professionals who suspect the occurrence of an adverse reaction to a drug or dental device have an obligation to communicate that information to the broader medical and dental community, including, in the case of a serious adverse event, the Food and Drug Administration (FDA).
